Panel discussion focused importance of data in creating U.S. stronger social care systems

It’s been four years since wellconnected introduced a new vision for the social care industry. The ‘allco’ platform is an expression of the belief in data to draw funders and nonprofit agencies closer together, creating a stronger safety net on behalf of the people they serve.
So the wellconnected team was beyond excited to spread this message last month at the HIMSS conference in Las Vegas, a massive international gathering around cutting-edge healthcare and IT.
wellconnected CEO Duane Conners was offered the opportunity to participate in a panel titled, “Data Matters: Elevating Health and Community Information Exchange,” to discuss our first-of-its-kind community information exchange in Chicago and the importance of leveraging data for the social determinants of health.
The panel was moderated by Stephanie Broderick, senior vice president of provider solutions at Clinical Architecture; Gregg Church, the CEO of 4medica; Waldo Mikels Carrasco, director of the Center for Health Information Sharing & Innovation; and Laura Nixon, vice president of interoperability at Clinical Architecture.
Conners described how wellconnected has been engineered to fit an agencies’ specific needs around intake and case management.
The platform coordinates that information in such a way that users can pull automated health information summaries, which are presented in layman terms and create crucial context for SDOH.
“Just like an HIE, ‘allco’ has a longitudinal record for social care,” Conners said. “The difference is ours is directly built into the case management suite. So if someone walks into a shelter, it’s a full-stack understanding of their history – if they ever needed transportation or childcare or have food insecurity – because all of those community agencies really need to work together.”
Conners continued: “If someone comes in for housing assistance, our users can see historically that he lost his job and he needed rent assistance that had fallen off. So he can step in and start working with those other providers to make sure that doesn’t happen again.”
