How a social care startup is integrating with Chicago’s health information exchange

As our work begins in earnest in Cook County, Chicago, the ‘allco’ team is tackling one of the most important aspects of this exciting project: integrating our system into 4Medica’s health information exchange.
This process is at the core of what makes this project unique. By taking our networked enterprise platform for nonprofit agencies and combining it with 4Medica’s expertise at building and maintaining health information exchanges, Chicago is making a concerted effort to improve care using social determinants of health. This resulting community information exchange will represent a deeper layer of data, with person-specific histories of care and systemic insight about the effectiveness of programs and allocation of resources.
The wellconnected engineering team is in full swing on the technical aspects of this integration, including how the two software platforms will interact and how information will be presented to workers on the ground.
Our business team is beginning to work with nonprofit agencies about how to use ‘allco’ and education around meaningful use of healthcare records. None of this works without buy-in from nonprofit and social workers on the ground, and so our software and company requires flexibility and customization to ensure that it’s part of the solution and not something holding them back.
Think of it this way: a person who comes into a homeless shelter will now complete an intake process guided by ‘allco’ (this process is customizable from agency to agency). The software will then create a summarized entry that includes previous visits to social care agencies and also doctor’s offices in Cook County. This will give agency workers context on the people they’re treating: how many times they’ve sought shelter, previous healthcare diagnoses, whether they have dietary restrictions and other person-specific needs.
On the other side, 4Medica’s healthcare exchange will provide the same information to doctor’s as they treat patients who have a history of seeking out social care. It’s the same goal. By creating summarized information about the whole person, caregivers spend less time gathering information and more time prescribing solutions.
