Second phase of Monmouth ACTS rollout is another step toward cutting-edge human services
Nearly 90% closed-loop referral rate sets standard in regional social care

The Monmouth ACTS program came from a recognition by Monmouth County officials that coordinated social care could create better outcomes for community members.
Using ‘allco’ as its electronic referral tool, the first year of the program resulted in a whopping 88% closed-loop referral success rate and assisted 1,600 community members with nearly 800 referrals. That means that more than 4 out of 5 community members who called the Monmouth ACTS Hotline for help with a referral to a community-based agency, that the care was delivered.
Before Monmouth ACTS the referrals were made but there was no documentation of their success and no systemic data on which to make decisions.
Now it’s time to take that effort up a notch and create a true community information exchange in Monmouth County, all toward the goal of a human services system that leads to better care for community members.
In the second phase of the program, which will begin this spring, each of the 60-plus agencies that work directly through ACTS will have full ‘allco’ licenses. That means they won’t just be receiving ‘allco’ referrals, they will be using our platform for intake, case management, record-keeping and reporting.
Again, ad hoc business practices will be replaced by a tool that’s built specifically to make lives easier in the social care industry, creating a digital ecosystem where care can be tracked from one agency to another. People will have social care histories that are analogous to electronic medical records, meaning that care providers can see where they’ve been treated before and use that context to get them what they need.
In the meantime, the ‘allco’ platform will surface system-level data and insight to Monmouth ACTS so that the entire spectrum of social care funding and care can be analyzed. It’s our belief that this level of perspective will drive strategic decisions and investments going forward.
The kind of data we’re generating is why allco is having conversations with counties, communities and funders across the U.S. to establish community information exchanges.
