wellconnected CIE project with Chicago homeless shelters set to go live

wellconnected is excited to bring a unique project in Chicago which could be the national model for how to leverage determinants of health and data-based practices in social care.

Now that project is almost ready for the light of day.

Working with five homeless shelters in Chicago and their clinical partners, a first-of-its-kind community information exchange (CIE) based on our ‘allco’ platform is set to go live in late September.

The goal is straightforward: give shelter staff and clinical teams a single environment for managing referrals, care coordination, and client data — with role-based access and appropriate consents. By late October, the pilot will be fully operational across the initial network.

This is a deeply empathetic system for community members who require care, giving both physicians and care providers context that will inform how they’re treated. It is the first systemic platform that helps doctors understand a patients’ social care history.

Over the next two months, our team is working closely with these providers to refine workflows, ensure HMIS compliance, and build familiarity with the allco platform. This includes hands-on support for staff as they begin to use shared digital records and a unified landing page in allco.

The allco team is laser-focused on creating an intuitive and user-friendly integration in Chicago. But this is also important validation for our broader mission to create collaborative, efficient delivery systems in social care across the U.S.

In parallel, wellconnected is reinforcing coordination with key stakeholders across the region. That includes 211 Metro Chicago and both local Continuums of Care — all Chicago and Alliance to End Homelessness in Suburban Cook County — who remain aligned on the goals of the pilot and its long-term potential.

This CIE pilot is built on allco, with standard customizations tailored to each partner’s workflow. The approach is designed to support adoption and ease of use across a range of organizational structures and staff roles.
