wellconnected has been selected participate in the upcoming Civitas Networks for Health Annual Conference, taking place September 22 to 25 in Anaheim, California. Our team has been invited to share the early outcomes of our major project in Chicago, which brings together shelter providers and clinical partners on a shared platform for social and health care coordination.

CEO Duane Conners will be part of a panel focused on cross-sector data infrastructure and local innovation. His remarks will center on the launch of the Cook County community information exchange (CIE) and the real-time integration of social and health data systems across the region.

The Cook County program is one of the first examples of its kind: a CIE environment operating in parallel with regional health information systems and serving frontline providers across multiple sectors. The pilot is built on allco, wellconnected’s case management platform, which is currently being implemented across five shelters and their paired clinical partners.

Civitas brings together community and data leaders working to improve health through information sharing, system alignment, and regional collaboration. We’re looking forward to joining those conversations — and to learning from peers across the country who are building new infrastructure for better care.
