$50B federal bill will transform rural healthcare. wellconnected is already steeped in that work

The recent announcement of the $50 billion Rural Health Transformation Program creates an opportunity for states to modernize how care is delivered, coordinated, and measured in rural communities.

• In Chicago, ‘allco’ has been adopted as an innovative, regionwide Community Information Exchange that supports homeless prevention in coordination with clinical care.
