wellconnected is excited to join with 4medica and the New Jersey Innovation Institute to carry the message of their successful partnership to the broader social care industry.
The three entities will present together at the upcoming Civitas 2025 Annual Conference, taking place Sept. 28-30 in Anaheim, California.
They will explain how their innovative work in Chicago and other communities uses data to systemize and track social care. Their coordinated system creates better processes, more transparency and ultimately better care for vulnerable community members.
