wellconnected in 2025: From regional results to national momentum

wellconnected was founded several years ago to introduce a more collaborative and efficient way of delivering regional social care.

We’re proud to say that 2025 was the year that vision came alive.

Across multiple regions, our work moved from pilots to production, from planning to live systems, and from local experimentation to models that can scale nationally. This work brought significant year-over-year increases in contracts, customers and revenue.

But at every stage of wellconnected's journey, our focus remains the same: Strengthening how communities connect people to the services they need.

Progress in 2025

In Chicago, wellconnected advanced a regionwide Community Information Exchange in partnership with public health leaders, technology partners, and frontline providers. This work brought together homeless shelters, community organizations, and care teams around shared data and coordinated workflows. The result is a stronger foundation for collaboration and a model for how large, complex regions can build connected social care infrastructure that actually works in practice.

Momentum also grew in New York and New Jersey. In New York State, allco began supporting community pharmacies with social needs screening, expanding the role pharmacies can play as trusted access points for care beyond the clinical setting. In New Jersey, the second phase of the Monmouth ACTS rollout continued to strengthen coordination across human services, reinforcing the value of shared systems and consistent implementation over time.

This year also brought meaningful recognition and growth for the team behind the work. Founder and CEO Duane Conners was named InfoTech WNY’s Tech Leader of the Year, reflecting both wellconnected’s progress and its leadership in the evolving social care landscape. We also added capacity with the appointment of Carolyn Montalbo as Community Experience Specialist, reinforcing our commitment to strong onboarding, clear communication, and long-term partner success.

The company's proven ability to implement community information exchanges in new communities to improve the social safety net got a national audience this August when wellconnected was invited to be a presenter at the Civitas 2025 conference.

The work ahead

Looking ahead, the opportunity is even bigger. Communities across the country are actively exploring Community Information Exchanges as a way to modernize the social safety net, reduce fragmentation, and improve outcomes. wellconnected is positioned to help lead that work.
