allco rollout begins in Chicago, with a focus on agencies combatting homelessness

wellconnected is excited to announce that it has begun integrations with social care agencies in Chicago, the initial step toward establishing a countywide community information exchange.

Spearheaded by the Illinois Public Health Institute with partners 4Medica and New Jersey Innovation Institute, the initiative will tie together community-based social care and healthcare in an innovative new way.

The new community information exchange will establish an apparatus for electronic health records in Chicago, but also set up a collaborative social care system that involves intake, record-keeping and referrals for community members. Just like an electronic medical record, physicians will have access to this information as they administer care.

wellconnected’s ‘allco’ platform is the case management platform that will manage the social care side of that interaction. The work began this January as wellconnected began integrating Metro Chicago 211’s service directory, creating a searchable directory of regional nonprofits, and full ‘allco’ onboarding with about a dozen social care agencies with a focus on homelessness and non-hospital medical care.

Where most nonprofit agencies use pen-and-paper or spreadsheets to track care, allco’s customizable tools give them intuitive intake tools, allowing them to keep records of whom they track and refer people to other nearby organizations. This layer of data is the foundation of understanding histories of care for specific people, but also the broader social care landscape in a given community.

Over the course of 2025, the current rollout will lead into a broader integration with the Cook County health information exchange, which will give doctors a much deeper understanding of the people they’re serving.

This collaboration between healthcare and social care in a specific community will allow care providers to pinpoint social determinants of health and to better help the people that need it. wellconnected is proud to be part of a movement that uses technology to create a better infrastructure of care in the U.S.
